By using this Transfusion Medicine Data Portal (herein "Data Portal"), you agree to the terms and conditions that governs the use of this Data Portal. If at any time, these terms and conditions are no longer acceptable to you, you must immediately cease all use of this Data Portal.

The Data Portal has been specifically developed for use by the BC transfusion medicine community as well as the Ministry of Health.
Conditions of Use
In consideration of being granted personalized access to this Data Portal, you agree:
1. That you are solely responsible for maintaining the security of your Password and ensuring that it is used for yourself only;
2. To change your password at least every 42 days;
3. That if you suspect that your password has been compromised to change it immediately and notify the PBCO;
4. Not to write down your User ID and password, store it in a file on your computer or permit anyone to observe you entering your User ID and password;
5. That your access to the Data Portal is through the Health Authorities' secured IT network;
6. To restrict access to the Data Portal to use only within Canada.
7. That your use of the Data Portal will be monitored and recorded in an audit log and reviewed regularly.
You acknowledge that your Password is unique to you and that PBCO does not have access to your Password. PBCO is not responsible for unauthorized access to accounts online or losses that occur as a result of you voluntarily disclosing your username and password, or the careless or improper handling, storing or disclosure by you of this information.
For additional protection, after 5 unsuccessful login attempts a user will be locked out. Users are automatically logged out after 30 minutes of inactivity.
PBCO may suspend or terminate User Access immediately without notice if it is determined that you have undertaken activities deemed to be inappropriate. This suspension will be for a period of time necessary to permit the thorough investigation of such suspended activity.
Use of Data Portal
PBCO is not responsible in any manner for direct, indirect, special or consequential damages, however caused, arising out of your use of this Data Portal and/or any web browser.
