In order to support compliance with the Health Canada blood regulations, PBCO collaborated with the BC Patient Safety Learning System (PSLS) to revise the Blood Transfusion or Blood Product - Safety Event Report Form to include events related to the BC Red Cell Redistribution Program

Typically, issues involving red cell redistribution are identified and reported by the Receiving Site but need to be investigated and followed up by the Shipping Site. 

Reporting errors and accidents through PSLS facilitates opportunities for continuous improvement. Health Authorities can also use the PSLS report functionality to comply with the Health Canada annual reporting requirement.
